Please complete this form once a student has attended a new student orientation AND has logged into their coursework. A student can't be enrolled unless they log into their coursework!
The student needs to have a green box on their attendance calendar for the day you select below. If the student needs a device before they can complete orientation please do the following:
- Establish contact with the family to ensure they want to enroll.
- Contact Scott Kimble at kimbles@go2qda.org by email and explain the situation.
- After the device is received, please complete the orientation process.
- After the orientation is completed and the student has logged into Maestro, complete this form.
I agree that the student listed here has completed orientation and logged into their coursework. This form will serve as the official enrollment notification. Your username will be logged and attached to this submission. Submissions are submitted to the Ohio Department of Education at 5PM and can't be changed after that time.
